Summary:
As a 72 bed post-acute program, the MDS Coordinator at Sante North Scottsdale plays a vital part in the facility's success.
The MDS Coordinator is responsible for assuring the timely completion of accurate assessments that meet Federal and State guidelines, and requires close attention to the PDPM nuances.
Working with the interdisciplinary team, the MDS Coordinator identifies guest acuity and needs, and is the guardian of the public quality metrics, VBP, and QRP.
He/she will facilitate the completion of Omnibus Reconciliation Act (OBRA) and Prospective Payment System (PPS) assessments to best reflect the care provided to the patients here at Sante North Scottsdale.
The ideal candidate has an eye for the big picture, and a passion for thoroughly capturing the care provided.
Essential Functions and
Responsibilities:
Complete assessments, Minimum Data Set (MDS) for all guests assigned.
Monitor completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines.
Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing.
Ensure accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned guests.
Promote and participate in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current / new regulations.
Maintain current knowledge of RAI Manual and MDS assessment process.
Schedule the ARD's (Assessment Reference Date) for Payment Assessments as needed, and as per applicable regulations.
Provide a schedule of ARD's and Assessment types weekly and as needed to the IDT in order to facilitate the timely completion of MDS sections and CAA's (Care Area Assessments) by each discipline.
Schedule all OBRA (Omnibus Budget Reconciliation Act) ARD's daily, monthly and PRN.
The schedule will be provided to the IDT (interdisciplinary team).
Schedule Admission, Quarterly and Annual Assessments.
Addendums to the schedule will be provided by the RAI Coordinator to the IDT as needed as it relates to Discharge Assessments and SCSA (Significant Change in Status Assessments).
Utilize the RAI Manual as a resource during the assessment coding process.
Coordinate the completion of MDS sections according to facility assignments.
Communicate to members of the IDT as needed based on timeliness of completion.
Ensure the transmission of PPS Assessments within 14 days of the completion date.
Ensure the transmission of Comprehensive assessments within 14 days of the Care Plan Completion Date.
Provide confirmation for the transmission file and review the printed initial and final validation report.
Facilitate the correction of any fatal errors immediately and retransmit the assessment until an accepted validation report is received.
Facilitate receiving Validation Reports timely, the RAI Coordinator will transmit as frequently (daily) as necessary to obtain timely validation of MDS acceptance into the QIESASAP data base and maintain validation reports.
Facilitate the completion and updating of guest care plans within the MDS Department and the IDT to reflect the guest's most current needs.
Identify guests in need of Interim Payment Assessment completion, or need a Significant Change in Status.
Participate in facility staff education as it relates to the RAI process, and the coordination of obtaining accurate documentation from direct care staff.
Compile data reports based on MDS data as requested by the Administrator and/or Director of Nursing.
Utilize the facility's software program and computer system to comply with federal requirements for completion and encoding.
Requirements:
Current, RN license or temporary RN license through the Arizona State Board of Nursing, or hold a compact RN license.
Must maintain valid licensure to practice as a RN; or current, LPN license or temporary LPN license through the Arizona State Board of Nursing, or hold a compact LPN license.
Must maintain valid licensure to practice as a LPN.
Must currently hold 3.
0 MDS Certification, OR have a minimum of one (1) year structured 3.
0 training experience.
Must have a minimum of one (1) year RN OR LPN experience.
CPR required.
Knowledge, Skills and Abilities:
Must have an active working knowledge of the RAI process in completing data collection duties.
Must have experience with completion of the CAA worksheets and application of the comprehensive care plan.
Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long-term care.
Able to relate positively, favorably and cooperatively with others, including physicians, associates, guests, visitors, and personnel of outside agencies and organizations.
Must have the ability to hold in confidence any information regarding guests, personnel, or any other matters which are confidential in nature.
